ABSTRACT

Objective:To investigate the value of preoperative MRI measurements of hamstring (semitendinosus+gracilis) tendon cross-sectional area in predicting intraoperative graft diameter for anterior cruciate ligament reconstruction (ACLR).Methods:A total of 265 preoperative MRI were retrospectively collected in the Third Affiliated Hospital of Southern Medical University from January 2013 to August 2020 for patients who underwent single-bundle ACLR of hamstring tendon. Patients were divided into a graft diameter≥8 mm group (129 patients) and a graft diameter<8 mm group (136 patients) according to intraoperative graft diameter. The cross-sectional areas of the semitendinosus and gracilis tendons were measured at the largest level of the femoral condyle on preoperative MRI cross-sectional images, and the two were summed to obtain the cross-sectional area of the hamstring tendon. The independent samples t-test was used to compare the differences in the cross-sectional area of each tendon between the graft diameter≥8 mm group and the graft diameter<8 mm group. The Spearman correlation analysis was used to assess the correlation between tendon cross-sectional area and intraoperative graft diameter. Multi-factor logistic regression analysis was used to screen the influence of tendon cross-sectional area on intraoperative graft diameter. The effectiveness of intraoperative graft diameter≥8 mm was assessed by plotting the receiver operating characteristic curves. Results:Intraoperative measurement of graft diameter was 7.5 (7.5, 8.0) mm. The cross-sectional area of the popliteal tendon was (21.4±4.6) mm 2 in the graft diameter≥8 mm group and (15.6±3.7) mm 2 in the graft diameter<8 mm group. Statistically significant differences were found in the cross-sectional areas of the semitendinosus, soleus and hamstring tendons between the graft diameter≥8 mm group and graft diameter<8 mm group ( t=-10.26, -10.29, -11.47, P<0.001). Intraoperative graft diameter was positively correlated with the cross-sectional area of the semitendinosus, gracilis, and hamstring, with correlation coefficients of 0.57 ( P<0.001), 0.58 ( P<0.001), and 0.62 ( P<0.001), respectively. Multi-factor logistic regression showed that popliteal tendon cross-sectional area was a predictor of intraoperative graft diameter (OR=1.45, 95%CI 1.32-1.59, P<0.001). The area under the curve for popliteal tendon cross-sectional area to predict intraoperative graft diameter≥8 mm was 0.838 (95%CI 0.792-0.885), with a critical value of 20.0 mm 2, a sensitivity of 0.581, and a specificity of 0.941. Conclusion:The measurement of the cross-sectional area of the hamstring muscle on preoperative MRI can predict the diameter of the autologous hamstring graft of ACLR.

ABSTRACT

Objective:To explore the application value of transrectal ultrasound images classification network model of prostate cancer based on deep learning in the classification of benign and malignant prostate tissue in transrectal ultrasound images.Methods:A total of 1 462 two-dimensional images of transrectal prostate biopsy with clear pathologic results(including 658 images of malignant tumor, 804 images of benign tumor) from 203 patients with suspicious prostate cancer(including 89 cases of malignant tumor, 114 cases of benign tumor) were collected from May 2018 to May 2021 in the First Affiliated Hospital of Jinan University. They were divided into the training database, validation database, and test database. And the training and validation database were used to train and obtain the intelligence-assisted diagnosis network model, and then the test database was used to test the network model and two ultrasound doctors of different ages. With pathologic diagnosis as the gold standard, the diagnostic performance among them was evaluated.Results:①The sensitivity of network model was 66.7% the specificity was 91.9%, the accuracy was 80.5%, the precision(positive predictive value) was 87.1%. The area under the ROC curve was 0.922. ②The accuracy of the junior and senior ultrasound doctors was 57.5%, 62.0%; the specificity was 62.0%, 66.3%; the sensitivity was 51.5%, 56.8%; the precision was 53.1%, 58.1%, respectively. ③The accuracy, sensitivity, specificity, precision of classification: the network model > the ultrasound doctors, the differences were significant( P<0.05); the senior ultrasound doctor>the junior ultrasound doctor, the differences were not significant( P>0.05). Conclusions:The intelligence-assisted diagnosis network model based on deep learning can classify benign and malignant prostate tissue in transrectal ultrasound images, improve the accuracy of ultrasound doctors in diagnosing prostate cancer. It is of great significance to improve the efficiency of screening for patients with high clinical suspicion of prostate cancer.

ABSTRACT

ObjectiveTo investigate the quality of life and demands of family members of patients with mental disorders in hospice care, in order to create a better medical care environment for patients to meet their demands. MethodsA total of 205 family members of patients with mental disorders in hospice care were consecutively sampled from two special psychiatric hospitals in Deyang city with convenient sampling method. The Chinese version of WHO Quality of Life-BREF (WHOQOL-BREF) and Critical Care Family Needs Inventory (CCFNI) were used to assess the quality of life and demands of family members. ResultsAmong the patients' family members, WHOQOL-BREF scored (68.08±9.98) in the physiological field, (63.82±9.39) in the psychological field, (70.73±12.61) in the social relations field, and (64.24±11.87) in the environmental field. Compared with the domestic general population, there were significant differences in other fields except the physiological field (t=3.066, 9.845, 16.109, P<0.01). In CCFNI, the score of condition assurance factor was (3.20±0.41), information support was (2.86±0.50), proximity to patients was (2.79±0.46), self comfort was (2.35±0.47), and medical staff support was (2.60±0.44). ConclusionThe quality of life of the family members of patients with mental disorders in hospice care may be higher than that of the general population, and their demands mainly focus on the condition assurance and information support.

OBJECTIVE@#To investigate the developmental characteristics of resilience in children aged 3-5, and to explore the relationship between temperament, parenting style and resilience.@*METHODS@#A total of 570 preschoolers aged 3-5 years in Hangzhou participated in this study. The children's teachers completed the assessment of the resilience scale of DECA-P2 (Devereux Early Childhood Assessment for Preschoolers Second Edition); the children's parents completed assessment of temperament questionnaire CBQ (Children's Behavior Questionnaire) and parenting style questionnaire PSDQ (Parenting Styles and Dimensions Questionnaire).@*RESULTS@#Totally 432 valid questionnaires were retrieved with a recovery rate of 75.79%. The levels of initiative and self-regulation of 5-y children were higher than those of children aged 3 or 4 (all <0.01); the level of attachment/relationship of 5-y children was higher than that of children aged 4 (<0.01); the levels of initiative and self-regulation of girls were higher than those of boys (<0.05 or <0.01). The negative affect dimension of temperament was negatively correlated with resilience (all <0.05), while the effortful control and authoritative parenting styles were positively correlated with resilience (all <0.05). The negative affect and effortful control were able to partially predict resilience of children through authoritative parenting style (mediating effect were-0.0143 and 0.0363).@*CONCLUSIONS@#Preschoolers aged 3-5 years with different age and gender show differences in resilience, and parenting styles may play a mediating effect between temperament and resilience.

ABSTRACT

Objective To investigate the correlation between the changes of fat and microcirculation in vertebral marrow and the intervertebral disc degeneration. Methods This was a cross-sectional cohort study. Based on the inclusion and exclusion criteria, all 82 patients were recruited, while 246 lumbar intervertebral disc (L2/3-L4/5) were studied. Each disc was assessed by using Pfirrmann grades. The chemical shift imaging (CSI) was performed to calculate the signal intensity ratio (SIR) of the corresponding upper and under vertebral marrow. And intravoxel incoherent motion (IVIM) imaging was performed to obtain the IVIM parameters of slow apparent diffusion coefficient (Dslow), fast apparent diffusion coefficient (Dfast) and perfusion fraction (f). At the same time, b values of 0, 600 s/mm2 were used to obtain the ADC value of each disc. The ADC values of disc, the SIR values and IVIM parameters of the upper and under vertebral marrow between the different segments and different Pfirrmann grading groups were compared using one-way ANOVA or non-parametric test. The correlation of the Pfirrmann grading and ADC value of disc with the parameters of the vertebral marrow were analyzed, respectively. Results Only the f value of the upper and under vertebral marrow showed significant difference between the different segments groups of L2/3 to L4/5 discs (F=5.351 and 8.482, both P<0.05). The ADC values of discs, the SIR value of the upper vertebral marrow and the Dslow value of the under vertebral marrow had significant difference between the different Pfirrmann grading groups (all P<0.05). The Pfirrmann grading was negatively correlated with the disc ADC values (r=-0.651, P<0.01), and was mildly and positively correlated with the SIR values of the upper and under vertebral marrow (r=0.238 and 0.266, both P<0.01). The disc ADC values had a slightly negative correlation with the SIR value of the upper and under vertebral marrow(r=-0.230 and-0.247, both P<0.01). Conclusions The changes of the SIR value and all IVIM parameters of the vertebral bone marrow were not very obvious with the increasing of the grading of the intervertebral disc degeneration, which may be not an effective supplement for the grading of intervertebral disc degeneration.

ABSTRACT

Aim To investigate the effect of p38 MAPK AS-ODNs on the expression of GLT-1 during the induction of brain ischemic tolerance induced by cerebral ischemic preconditioning (CIP).Methods Eighty healthy male Wistar rats with permanent occlusion of the bilateral vertebral arteries were randomly divided into 6 groups: ①Sham group (n=10);②CIP group (n=10);③ischemic insult (Ⅱ) group (n=10);④CIP+Ⅱ group (n=10);⑤p38 MAPK AS-ODNs+CIP+Ⅱ group (n=30);⑥p38 MAPK S-ODNs+CIP+Ⅱ group (n=10).Group ⑤ was divided into 5 nmol, 10 nmol and 15 nmol subgroups according to the dose of p38 MAPK AS-ODNs (n=10).The dose of p38 MAPK S-ODNs was 15 nmol.All the rats were sacrificed 6 h and 2 d after the sham operation or the last time of global brain ischemia reperfusion.Western blot and immunohistochemistry analysis were used for detecting the expression of p-p38 MAPK and GLT-1 protein.Results CIP moderately up-regulated the expression of p-p38 MAPK and significantly up-regulated the expression of GLT-1 protein, inhibited the excessively up-regulation of p-p38 MAPK and the down-regulation of GLT-1 induced by ischemic insult.p38 MAPK AS-ODNs significantly inhibited the up-regulation of p-p38 MAPK and GLT-1 protein in a dose-dependent manner during the induction of brain ischemic tolerance by CIP.Conclusion p38 MAPK AS-ODNs inhibit the up-regulation of GLT-1 during the induction of brain ischemic tolerance induced by CIP.

ABSTRACT

Objective To investigate the value of CT spectral imaging in differential diagnosis of atypical pheochromocytoma and adrenal adenoma.Methods The energy spectrum CT data of 13 cases of atypical pheochromocytoma and 15 cases of adrenal cortical adenoma confirmed by surgery pathology were retrospectively analyzed.All images processed by spectrum software.The CT value of different energy levels (40-140 keV) in arterial,venous and delayed phase,energy spectrum curve and spectrum curve type,base material of concentration (water-iodine,grease-iodine,blood-iodine) were obtained,and the CT value of different energy were standardize in the same period.Data were compared with samples t test.Results For arterial phase,the standardization CT values of pheochromocytoma on monochromatic images of 40 keV,70 keV to 140 keV had statistical difference with those of adenoma (all P<0.05).For venous phase,the standardization CT values of pheochromocytoma on monochromatic images of 70 keV to 140 keV had statistical difference with those of adenoma (all P<0.05).For delayed phase,the standardization CT values of the two tumors on monochromatic image of 40 keV to 140 keV had significant difference (all P<0.05).Both the energy spectrum of the three periods of type curve was falling,pheochromocytoma always located above compared with adenoma.The standardization concentration of base materials including water (iodine),fat (iodine) and blood (iodine) of pheochromocytoma were higher than those of adenoma in three phases (all P<0.001).The standardization concentration of base materials including iodine (water),iodine (fat) and iodine (blood) had no significant difference in three phases between pheochromocytoma and adenoma (all P> 0.05).Conclusion Atypical pheochromocytoma and adrenal adenoma have different spectral curve and spectrum characteristics of the material content.Spectral CT imaging provides a method of multiple parameters to identify atypical adrenal pheochromocytoma and adenoma.

ABSTRACT

Objective To evaluate the value of spectral CT in risk classification of gastrointestinal stromal tumor (GIST).Methods Forty-one patients with GIST were collected,including 1 3 in high risk,1 2 in medium risk,1 1 in low risk and 5 in very low risk. All of the patients underwent plain and triple phases contrast enhanced CT with spectral CT scanning mode.CT value of 70 keV monochromatic images,the slope of spectral curve and normalized iodine concentration (NIC)were measured and calculated.Then, data was correlated with the risk of GIST.Results ①The CT value of 70 keV monochromatic images was negative correlation in the arterial phase (r=-0.173),positive correlation in delayed phase (r=0.552),and no correlation in venous phase with the risk of GIST.②The slopes of spectral curves in delayed phase were significantly different in GISTs with the different risk (χ2= 6.641,P= 0.036).③The NIC values of GISTs with different risk were significantly different in the triple phases (F=3.646,χ2=6.046,F=57.233,P=0.035, P=0.049,P<0.001),and correlated with the risk of GIST (r=-0.564)in the delayed phase.Conclusion Quantitative paremeters of spectral CT imaging such as the CT value of 70 keV,the slopes of spectral curves and iodine concentration are valuable for the risk classification of GIST,and can be used as a preoperative reference.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the role of Src family kinases (Fgr, Hck, Lyn) and the major protein kinase C substrate SSeCKS in non-alcoholic steatohepatitis (NASH) and determine the possible mechanism regulating differential expression.</p><p><b>METHODS</b>Kupffer cells were stimulated with CCL4 and effect on SSeCKS, Hck, Fgr, and Lyn expression was detected by real-time reverse transcription-PCR. Male Sprague-Dawley rats were used to create a NASH model by feeing a high fat diet. The modeled rats were divided into a model group and a normal group. After sacrifice, the extent of hepatic steatosis and inflammation was assessed, and the expression levels of SSeCKS and Hck, Fgr, Lyn were detected by immunohistochemical staining.</p><p><b>RESULTS</b>Expression of Lyn and Hck was decreased in the CCL4-stimulated Kupffer cells and the change in expression level was positively associated with levels of inflammatory stimuli (P < 0.01). The change in expression of SSeCKS in the CCL4-stimulated Kupffer cells was negatively correlated with inflammatory stimuli (P < 0.01). Fgr expression was very low in the unstimulated Kupffer cells and was not affected by the exposure to inflammatory stimuli. The number of inflammatory cells in the liver tissues of rars were negatively correlated with expression of Lyn, Hck and SSeCKS (P < 0.01), with low negative correlation for Lyn (r =-0.398, P < 0.01) and moderate negative correlation for Hck (r=-0.508, P < 0.01); the Lyn and Hck expression levels were highly positively correlated (r =0.942, P < 0.01).</p><p><b>CONCLUSION</b>Src family kinases (Lyn, Hck and Fgr) and SSeCKS are involved in development and progression of NASH, and their differential expression patterns are associated to a certain extent. The factors may represent potential targets of therapy for NASH-related inflammation.</p>

ABSTRACT

ObjectiveTo explore the value of differential diagnosis between pigmented villonodular synovitis (PVNS) and rheumatoid arthritis (RA) in active stage with clinic and ultrasound. MethodsWe analyzed retrospectively the characters of clinic and ultrasound of sixteen patients with PVNS and sixteen patients with rheumatoid arthritis inactive stage.ResultsIn aspects of patient’s age (T=185), pain scores (T=136) history of trauma (P=0.029), functionally uncompensated conditions (P=0.001) type (P=0.000) and sum (P=0.000) of involved joints、bright spot-echoes in hydrops (P=0.001) tendons nearby involved (P=0.001) and the Adler’s grade of Doppler ultrasound blood flow (P=0.009), the differences between PVNS and RA in active stage had statistical significance (P0.05).ConclusionThe clinical characters including Patient’s age, pain scores, history of trauma, functionally uncompensated conditions, and the characters of ultrasound including type and sum of involved joints, bright spot echoes in hydrops, tendons nearby involved and the Adler’s grade of Doppler ultrasound blood flow, are helpful for the differential diagnosis of PVNS and RA in active stage.

ABSTRACT

Objective To explore the value of ultrasonographic diagnosis of deltoid contracture (DC). Methods A total of 37 children with bilateral asymmetric shoulder were reviewed retrospectively, and 8 cases with 9 shoulders were surgically confirmed DC. The results of the ultrasonography and surgery were compared. Results The sensitivity of ultrasonographic diagnosis of DC was 88.89% and specificity was 100%. No statistically significant difference of contracture thickness measurement was found between surgery and ultrasonography. Ultrasonographic findings of DC in children included hyperechoic areas (7 cases) or hypoechoic area (1 case). Conclusion Ultrasounogrphy is an effective method for diagnosis of DC in children.

ABSTRACT

One yeast strain, which was isolated from 256 natural samples, was found to be able to utilize D-xylose effectively. On the basis of assimilation physiological and molecular biological tests, the yeast strain was identified as a strain of Candida tropicalis. Furthermore, metabolic engineering breeding strategy was applied to change the metabolic flux in order to increase ethanol productivity. In this study, the C. tropicalis was used as the host strain and the plasmid pYX212-XYL2, which was formerly constructed for over expression of XYL2 gene encoding xylitol dehydrogenase (XDH) from Pichia stipitis, was used as the backbone of the recombinant vector. A hygro gene was inserted into downstream position of XYL2 gene, meanwhile, the result plasmid pXY212-XYL2-Hygro transformed into C. tropicalis by electroporation. Thus, a recombinant yeast C. tropicalis XYL2-7 was obtained through hygromycin B resistance screening and its specific XDH activity was 0.5 u/mg protein, which was 3 times more than that of the parent strain. Additionally, the recombinant yeast was applied in the fermentation of xylose. Compared with the parent yeast, it was concluded that the xylitol yield in the broth decreased by 3 times, however, the ethanol yield increased by 5 times. The feasibility of ethanol production from xylose by C. tropicalis was firstly studied in this paper. These research results are helpful to advance the bioconversion of renewable resources (e. g. straw, wheat bran, and husk) to fuel ethanol.

ABSTRACT

Objective To investigate the correlated clinic factors of prostecdtive efficacy and recurrence in chronic hepatitis B patients who got the combinated fully respond to Lamivudine.Methods To divide the chronic hepatitis B patients who got the combinated fully respond to Lamivudine into two groups,according to whether the curative effect was consolidated after the withdrawal of lamivudine therapy for two years.Group A was consolidated and group B was repeated.To compare the two groups correlated clinic factors.Results Consolidated ratio of prostecdtive efficacy was 64.71% in the first year,then it dropped to 55.88% in the second year.There was apparent difference between two groups which included:ages,total months of treatment,serum HBV DNA before and after treatment,ALT,time to HBeAg seroconversion and consolidation.Particularly is time stoped a medicine,the pro-s 1 Ag was negative,the HbeAg fixed amount is a 0 Ncus/ml,two all match 33 example,only 1 while stopping a medicine for 12 months relapse,two years long-term curative effect consolidation rate reaches to 97%,.But come short of twos to all match 35 example,29 HBVDNA rebound,relapse of illness,two years long-term curative effect consolidation rate reaches to 17.14% only.(P=0.0000).Conclusions To achieve the withdrawal indication after long-term antiviral therapy with nucleoside according to The Guideline of Prevention and Treatment for Chronic Hepatitis B,it still must refer to that the pro-s1Ag was negative.whether the quantitated index of HbeAg had achieved to 0Ncu/ml,otherwise,it was necessary to extend the course of treatment of antiviral therapy.The extensive course of treatment was very safe,because the ratio of mutation of YMDD and some other virus was tiny.

Objective:To explore the concentration of monoamine neurotransmitter in serum and psychiatric symptoms such as anxiety and depression in infertility patients with and without PCOS(No-PCOS),and the relationship between them.Methods:30 infertility with PCOS were selected as experimental group(EG) and 30 infertility patients without PCOS as control group(CG).They were administered Symptom Checklist(SCL-90).Their monoamine neurotransmitter in serum were determined including norepinephrine(NE),3-methoxy-4-hydroxyphenylglycol(MHPG),5-hydroxytryptamine(5-HT),5-hydroxyindoleacetic acid(5-HIAA),dopamine(DA),homovanillic acid(HVA) and dihydroxy-phenyl acetic acid(DOPAC).Results:The findings indicated that the scores in anxiety and depression subscales of SCL-90 were significantly higher in the PCOS group than those in the No-PCOS(P

ABSTRACT

Objective To evaluate the results of the treatment and prevention measures for pathological dislocation of the hip following septic arthritis in the newborns. Methods Twenty cases undergone open reduction were studied restropectively. In addition, greater trochanterplasty was penformed in 15 cases, acetabularplasty in 14 cases, greater and lesser trochanterplasty in 2 cases. Rusults A successful rate of reduction achieved in 90% of the patients; however, central reduction was achieved in only 38.8% . 20 patients were followed-up for 6 months to 12 years. The stability and motility of the hip gradually improved in majority of the patients. The cases with severe destruction followed by absence of femoral head and neck had unsatisfactory results. Shortening deformity were found in six cases, and subluxation, dislocation, rigidity of the hip in one case each. Conclusion The greater trochanterplasty had a good result for reduction and maintaining the position. Early diagnosis and early open drainage were key measures for prevention of pathological dislocation of the hip resulting from the acute septic arthritis in the newborn.
